Instructions for Submitting Your Physical Form

1. Download the Physical Form
   If you need a copy of the physical form, download the IHSAA Physical Form below and print it.

2. Complete the Physical
   Schedule an appointment with a licensed health physician to complete and sign the physical form.
   A parent or guardian must also fill out and sign all required sections of the form.

3. Upload the Completed Form
   Once your physical is fully completed and signed, you or your doctor's office can click HERE to upload the form.
